SUMMER CAMP FORMS
Registration Forms
Must be completed by Parent/Guardian. Person signing camper out of camp must be listed on form and bring photo ID on last day to camp.
Must be completed BY A PHYSICIAN for both prescription AND non-prescription medication that camper may/will take at camp.
Required and Medical Forms
Must be brought to camp with camper
Camper Medical Form is required for all campers to attend camp.
